  1. Use one form per person and complete all questions. incomplete forms will be rejected!
  2. If you wish to work with a friend or spouse, please complete that portion of the form but you both must submit forms.
  3. Volunteers under the age of 18 must have parental release completed before assignments can be made!

NOTE: Scheduling is made on a first come first served basis. We will TRY to place you where you choose, but please keep in mind this is not always possible!
